PASSPORT
To apply for a passport, every applicant must appear in person at the Embassy for biometric data collection (photo, fingerprints) and signature. In addition, the following documents must be submitted:
Documents required for passport issuance:
For individuals aged 18 and above:
- Original and copy of the identity card and passport;
- Application form (only sections 1.1 to 1.8 must be filled and signed);
- Two passport photos (3x4 cm);
- For males aged 18-30: original and copy of the military ID or a certificate confirming deferment status.
For individuals under 18 years of age:
- Original and copy of identity card and passport – for first-time applicants, the original and a copy of the birth certificate (with Azerbaijani translation);
- Application form (only sections 1.1 to 1.8 must be filled and signed);
- Two passport photos (3x4 cm);
- Written consent from both parents confirming they do not object to the passport issuance. This consent must be completed in advance and personally signed at the consular section by both parents. If one or both parents cannot appear in person, their consent must be notarized. The consent form must be prepared in Azerbaijani;
- Original and copy of both parents’ identification documents (passport or identity card);
- For first-time applicants under age 15 without an identity card: a medical certificate indicating the child’s blood type;
- If the applicant under age 15 does not have an identity card, the passport may be issued based on a Personal Identification Number (PIN). In this case, the legal guardian must first apply at the Embassy to request a PIN.
Note 1: When receiving a new passport, the previous one must be returned to the Embassy if it has expired, been damaged, or is full.
Note 2: In case of a lost or stolen passport, a document issued by the local police confirming the report must be submitted.

Note: All state fees must be paid by bank transfer to the account listed above. Cash payments are not accepted at the Embassy. Payment receipts or bank statements must be printed and submitted to the Embassy.
Attention: In addition to the above fee, the relevant state fee for passport issuance based on the applicant’s age must be paid online via https://www.asanpay.az/intro/home (select “Ministry of Internal Affairs” and enter the PIN), or in person by the applicant’s close relative/representative in Azerbaijan at the Passport, Registration, and Migration Department. The original or a copy of the payment receipt must be submitted along with other documents to the consular section.
